New center navigates patients and their families through the Healthcare Maze

Winthrop University Hospital has established The Center for the Advanced Care of Chronic Conditions as part of the hospital’s continuing commitment to address changing and emerging healthcare needs. The Center specializes in treating a range of chronic health conditions. “Chronic” refers to a lifelong condition, as opposed to an acute illness, which is generally short term. The Center utilizes a nurse navigator to assist each patient in developing a health and wellness plan, tailored to their needs that result in building an individualized clinical team. The Center gives the navigators the goal of seeking or confirming diagnoses and a care plan, including a review of current treatments, diagnostic testing, and medications. The team will explore options not only at the Winthrop’s campus, but also at facilities and physicians’ offices on Long Island and in Manhattan. Wellness and life-style modification often provide significant benefits for all patients. There is no fee for navigation services, and the team aims to reflect Winthrop University Hospital’s concern for the healthcare needs of every community. The Center offers coordinated treatment, open communication with the patient’s medical team and endless support and encouragement.
